New Patient Information

What to Bring to Your Appointment

 

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment could be done the same day of the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. Please assist us by providing the following information at the time of your consultation:

  • Patient's Health Insurance Card(s)

  • Photo ID

  • Primary Care Doctor's name and telephone number

  • Your referral slip, if applicable (If Required By Your Insurance Carrier)

  • A list of medications you are presently taking

  • Information about patient's medical and surgical history

  • Any Previous X-Rays, MRIs or Test Results Relating To Any Past Foot Health Conditions

  • If you have medical insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

You can save some time before your first appointment by completing your new patient paperwork from home. You will need the Adobe Reader Software in order to view/print the forms. You can download it for free by clicking here. You'll find also button links to the forms to fill out below.


Patient Forms

 

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).


X-Rays

 

If your previous doctor has taken recent X-Rays of the affected area within 6 months prior to the consultation, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.

We know how important it is to keep all of our patients' out-of-pocket expenses reasonable and affordable. We're always happy to submit all appropriate insurance forms for you, and help you recover the maximum possible value from your benefits. Usually, payment will be due at the time your service is rendered, unless you've already made other, advance arrangements. Our fees and pricing are comparable to any Miami podiatry offices. Your treatment will never be compromised in any way to meet the arbitrary "usual and customary" fees that insurance companies often impose.

For your best convenience- and to provide the best feasible treatment- we accept all major credit cards (such as Visa, MasterCard, American Express, Discover, and others), cash, and Care Credit. Care Credit offers no-interest plans of 6 months or 12 months, or simple fixed monthly payments that will include the interest for up to 5 years. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. 

Reimbursement from insurance is sometimes a lengthy and complicated process. If we haven't received the insurance portion of your claims within 45 days, this balance will become your responsibility. In addition, please note that some or all of our provided services to you may be categorized as non-covered services, and thus not considered "reasonable and necessary" under the policies of your current insurance provider. A monthly statement will be sent to you. It's usual for an insurance company to respond in under 4 to 6 weeks. If your statement is inaccurate in regards to your insurance payment within that time frame, please let our office know immediately. It is your responsibility to pay all the balance that may be left after insurance has covered its share. We appreciate your prompt payment.
